Summer time means well check season for our older, school aged patients. If you have not already done so, please call as soon as possible for an appointment. Dr. Grady and Dr. Owens have availability starting in late July; our nurse practitioner Jessica Thomas has appointments available immediately.
It is very important that you bring along school and camp forms needing a doctor’s signature to the well check appointment. Before arriving, please complete as much of the forms as you are able.

The packet is 12 pages long. Please bring just the last two pages. Please complete the health history BEFORE your child’s appointment. The other form is the actual physical form. Bring both the health history and physical form to your child’s well check appointment.
Per AISD, physical exams must be dated AFTER April 15 and are good for one school year. If by chance your child has already had a well check before the April 15th date, you can schedule a “sports physical” instead, with an out of pocket expense of just $25.
Camp forms vary greatly, please read carefully and complete your part. Forms can be completed at the time of the child’s well check. If you need forms completed at other times, please mail to or drop off at our office and allow one week for completion.
