If your child needs a school physical or a camp form completed, it is imperative that you as the parent complete all areas of the form indicated for “parent”. Please complete all parts that you can: name, address, birthday, etc. There will likely be questions about medical history and medications that you must complete. The doctor’s signature attests to the parent’s answers, so the answers must be there!
Also, you can fax, email, mail or hand carry forms into our office. We can mail or fax your child’s forms to school or camp if you provide a signed medical records release form (click “forms” from our home page). Due to HIPAA regulations, without that records release, we can only return the forms to you, not a school or camp.
It’s now the time of year when forms will begin to flood our office, please help us expedite their completion by completing your parts of the forms. Happy camping!
